NOTICE OF HEALTH PROBLEMS

We have a registered nurse on duty from 8:00 a.m. to 4:30 p.m. daily.  In the event of serious injury, parents will be called.  Please update changes in your phone number for home, cell, and work with our office.  Minor cuts and scrapes will be taken care of by our nurse, or an adult staff member.  Please call the school office to report any contagious disease your child may have contracted.

IMMUNIZATION REQUIREMENTS

All children enrolled at FBCA must meet the immunization requirements of the State of Missouri.  By the time children enter kindergarten at age 5, they should have 5 DTP; 4 OPV; 2 MMR; 4 Hib; 3 Hep B and 2 Varivax (chicken pox vaccine) effective with the 2010 school year.

HEALTH CONDITIONS THAT REQUIRE ABSENCE

For the protection of all students at FBCA, anyone with the following symptoms will be sent home:

• A temperature of 100° or above
• Children are to remain home until they are fever free (without medication) for 24 hours
• Vomiting
• Diarrhea
• Any contagious disease
• Head live or nits - children with this condition must be checked and approved by the school nurse before they may be readmitted
• If your child’s illness requires an antibiotic, he or she must take the medication for 24 hours prior to returning to school

ADMINISTRATION OF MEDICATIONS

Only medicines that cannot be given at an alternate time should be sent to school.  All prescription medication sent to school must be in the original container and include:
     The child’s name
     Dosage to be given
     Schedule for giving the medication
     Expiration date
A “Request for Medication to be Dispensed” form must be completed when you drop off the medication at the office.   

FOOD ALLERGY NOTICE

For the safety of our students, all treats served at school MUST be STORE-BOUGHT.  This applies to birthday celebrations, classroom parties and special events.  Because of potentially life-threatening allergies to peanuts, treat MUST NOT contain nuts or nut products.  Please abide by these rules and check the ingredients list of the items you purchase.
